1. Print the Digital Certificate Application Form
2. Fill out all of the required information including signatures.
3. Attach a copy of ONE of the five following reference documents:
- Photocopy of your College of Pharmacists License Identification Card.
- Photocopy of your College of Pharmacists License Certificate.
- Official letter (use our template: in English, en Français) signed by an Officer of the Hospital Organization (CEO, CFO or COO) stating you are approved on behalf of the hospital organization to sign for Controlled Substances orders and receipts.
- Official Declaration of Guarantor Letter signed and completed by an approved Guarantor
(For a QPIC (Qualified Person in Charge) or Practioner).
- In-person verification with valid government-issued photo identification at:

We will process your application including verification with your Hospital and College of Pharmacy.
We will send you an email with a link and instructions to download the app.
*Note: Most Hospital IT departments require that they be involved in the installment process, so please ask for their assistance when you get the email.
After the installation process is complete we will send you an email with your User Name.
We will call you directly with your Password.
(We will not leave this with any other person or on Voice Mail.)
You are now ready to sign orders.
**Important Note: Do not share your User Name and especially Password with anyone under any circumstances, if you do you will be held liable for that other parties actions and may be subject to professional, legal or criminal consequences.
